My cousin, who is morbidly obese, required a nurse to help him with this. After problems with his health insurance arose, he lost the nurse, and was eventually unable to wipe himself or have himself wiped. This led to a trip to the hospital to deal with the massive staph infection that had spread over his lower body. I believe he got some skin grafts too.
